What can Gwennol take on?

Bespoke commissioned support packages including urgent 1:1 and 2:1 arrangements, placement stabilisation, community-based support, bridging support while a placement is arranged, step-down and transitions. We also deliver family support and edge of care work, supported and supervised contact, young people's outreach, assessments, and training and reflective supervision. We do not take on work that would require a registration we do not currently hold.

What information do you need from us?

The referral, current information about the situation, known risks and safeguarding considerations, the support and staffing arrangement you think is required, the location, and the outcomes you need. Please provide only the information needed for initial screening — do not send full case files or court bundles through the website form. If further records are required, we will agree an appropriate secure method for sharing them.

How quickly can a package be considered or mobilised?

We will confirm feasibility and mobilisation requirements following referral review. Mobilisation depends on the staffing ratio, the practitioner skills required and availability in that specific area, so we will not quote a standard timescale before we have read the referral.

Who manages the service?

Gwennol. For packages we are commissioned to deliver, and within the scope of the contract and our current regulatory position, we manage mobilisation, practitioner allocation, the rota and contingency cover, support planning, risk assessment, safeguarding processes, practitioner supervision, records, incidents, reporting and review. You have a named contact throughout.

How is risk handled?

Risk is assessed before delivery starts and documented in the package design. It is reviewed through the life of the package. Incidents are recorded, escalated and followed up, with referral into statutory safeguarding processes where thresholds are met.

What reporting do we receive?

Format and frequency are agreed at design stage, before delivery begins — daily, weekly or monthly as required. Records are kept from the first session. You should never have to chase us for information about a package we are running.

How do reviews work?

Reviews are scheduled as part of the package design. They look at risk, progress against the intended outcomes, and whether the package should continue as it is, step down, increase or change shape.

What happens when needs change?

The package is not fixed for its lifetime. If the need or the risk changes, we discuss the staffing ratio, hours and skill mix with you and agree a revised arrangement rather than carrying on with a model that no longer fits.

How are practitioners selected?

Against the needs and risks in the referral, the skills and experience the work requires, continuity for the person being supported, geography and availability. Practitioners are briefed on the plan, the risks and the reporting standard before delivery begins.

What geographic areas do you cover?

South Wales is our current core region, including Cardiff, Swansea, Newport and surrounding authorities. We are planning expansion across the South West of England, particularly Bristol and South Gloucestershire. Coverage for any individual package is confirmed at referral review, because it depends on practitioner availability in that specific area.

Phase 1 boundary: these services do not include personal care, prompting or reminding a service user to eat, wash, dress or take medication, medication administration, or accommodation combined with parenting assessment. Referrals are screened before acceptance.
