A placement at risk of breakdownAdditional support around a child or young person to help hold a placement that is under strain.
Urgent 1:1 or 2:1 supportA defined package with the staffing ratio set by risk and need, not by whoever is available.
Placement stabilisationShort-term intensive work at the point of crisis. The support is the intervention, not just the cover.
Bridging while a placement is arrangedCommunity-based support that keeps things stable during the search.
Step-down from residential or secure provisionSupport that reduces as independence grows, with the young person's goals at the centre.
Intensive family supportFocused work to prevent escalation and reduce the risk of family breakdown.
Outreach and transition for young peopleCommunity-based support built around engagement, independence, education and planned transitions.
Community support where residential is unavailable or unsuitableA managed alternative for the period between referral and placement.

Who delivers it

Who delivers it

Gwennol builds and manages practitioner teams based on the needs and risks in the referral. Three practitioner types are used across support packages.

Support practitioner

Direct delivery of 1:1 and 2:1 support, outreach and family work.

Senior practitioner

Same scope as a support practitioner, with additional experience for more complex cases.

Qualified social worker

Direct delivery where the role requires input from a registered professional.

Staffing ratios of 1:1, 2:1 and other arrangements are set by need and risk at the design stage. Waking-night cover is available where required. See rates.

Written support planAgreed with the commissioner before delivery starts and updated as things change.
Risk assessmentDocumented risk assessment, reviewed through the life of the package.
Daily recordsContemporaneous records of delivery, kept to an agreed standard.
Incident managementRecording, escalation and follow-up of incidents throughout the package.
Named management contactA single point of contact for the commissioner throughout.
Practitioner supervisionNamed supervision arrangements and a stated frequency for practitioners on the package.
Progress and review reportingAt the format and frequency agreed at the design stage.
Scheduled reviewsOf risk, progress and outcomes, and whether the package should step down or change.
Phase 1 boundary: these services do not include personal care, prompting or reminding a service user to eat, wash, dress or take medication, medication administration, or accommodation combined with parenting assessment. Referrals are screened before acceptance.
