We screen every referral against our current service scope and operating boundaries before we accept it. That is deliberate — it is better for everyone that we say no early than accept something we cannot deliver safely.

We will confirm feasibility and mobilisation requirements following referral review.

Please do not attach case files. This form is for initial referral information only. Do not send court documents, full case files, or unnecessary identifying or special-category personal data through this form. If further records are required, we will agree an appropriate secure method for sharing them.
Phase 1 boundary: these services do not include personal care, prompting or reminding a service user to eat, wash, dress or take medication, medication administration, or accommodation combined with parenting assessment. Referrals are screened before acceptance.
